Because the nature of this training it may be necessary for Enhanced Tactical LLC to perform a background check. The information you provided above will be used for

PHOTOGRAPHING AND VIDEO RECORDING PERMISSION
I am aware there may be photos and video recording during classroom, range and shooting. These photos and videos may be shared on the web, social media, newsletters / print, and or other training classes.

Note: if you opt out, photos and video recording will still occur but when published your face and identifying features will be blurred out.

PHYSICAL CONTACT ACKNOWLEDGEMENT
Instructors do their best not to touch students. But there are times when it is necessary for an instructor to physically touch a student. If there is not a safety issue and time is not of the essence the instructor will typically ask permission. If there is a safety issue or an immediate need, the instructor without asking may touch the student in an appropriate manner to remediate the issue. Typically, this contact will be on the hands, arms, or shoulders.

I hereby waive and release, indemnify, hold harmless and forever discharge Enhanced Tactical LLC, its agents, employees, officers, directors, affiliates, successors and assigns of and from any and all claims, demands, debts, contracts, expenses, causes of action, lawsuits, damages and liabilities, of every kind and nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to my or my child’s participation in any of the events or activities conducted by, on the premises of or in the field with, or for the benefit of, Firearms Training, provided that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful or malicious misconduct.

I understand that the activities that I or my child will be participating in may be dangerous and may cause serious or grievous injuries, including bodily injury. On behalf of myself, my heirs, assigns and next of kin, I waive any claims for damages, injuries and/or death sustained to me or my property, that I may have against the aforementioned released party(ies) to such activity.

By this Waiver, I assume any risk, and take full responsibility and waive any claims of personal injury, death or damage to personal property associated with Enhanced Tactical LLC concerning Firearms Training.

I have read, understand and fully agree to the terms of this WAIVER AND RELEASE. I understand and confirm that by signing this WAIVER AND RELEASE I have given up considerable future legal rights. I am signing this Agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me. My signature is proof of my intention to execute a complete and unconditional WAIVER AND RELEASE of all liability to the full extent of the law.
